Care & Support Worker

Reporting to:         Team Manager

Key Tasks

·        Attends to care and support tasks as agreed and in line with the service user’s plan, all of which are aimed at enabling the service user to maintain their independence within their own home or community, whilst working towards identified outcomes.

·        Supporting service users around their mobility and independence, helping users in and out of bed, bath, wheelchair and chairs following agreed manual Handling and Risk Assessments.

·        Where assessed, undertakes the monitoring, prompting and overseeing of service users medication, but only under the direction of a Supervisor and following company guidelines, policies and procedures.

·        Provide personal care such as washing, bathing, dressing, feeding and toileting assistance to the Service Users and will undertake manual handling duties as required.

·        Maintain necessary records such as Service Users care plans, and contribute to reviews of customers’ needs as directed.

·        In most circumstances, unless otherwise stated, access to a vehicle & the ability to drive is required.
