Trident Healthcare have an ongoing need for compassionate, motivated, and skilled Community Nurses’ to join a dedicated community nursing team in Sussex. This role offers the opportunity to deliver high-quality, person-centred care to patients in their own homes and community settings, supporting independence, rehabilitation, and chronic condition management.
You will be part of a multidisciplinary team, playing a key role in the delivery of holistic nursing care and supporting patients to live safely and comfortably in their communities.
Key Responsibilities:
- Deliver skilled, evidence-based nursing care to patients in their homes or community clinics
- Perform wound care, catheter and continence management, medication administration (including insulin and injectable therapies), and palliative/end-of-life care
- Assess, plan, implement and evaluate individualised care plans in partnership with patients, carers, and families
- Promote self-care and independence where appropriate
- Collaborate closely with GPs, social care teams, physiotherapists, occupational therapists, and other allied health professionals
- Maintain accurate and timely clinical records in line with NMC and local standards
- Use clinical judgement and escalate concerns appropriately to senior colleagues or other professionals
- Support junior staff, students, and healthcare assistants as required
- Contribute to service development, quality improvement initiatives, and patient safety strategies
Essential Requirements:
- NMC Registered Nurse (Adult)
- Strong communication and interpersonal skills
- Full UK driving licence and access to a vehicle (essential for community visits)
- Ability to work autonomously and as part of a team
- Knowledge of safeguarding, infection prevention, and health promotion principles
