Job Description: Trident Healthcare Solutions is seeking an experienced Band 6 Occupational therapist for a locum assignment based within the Intermediate Care Team at an NHS Trust in Hull. The successful candidate will independently manage a caseload of adult patients.

 

Responsibilities:

• Manage a caseload of intermediate care patients

• Complete assessments, treatment plans, discharge planning and onward referrals

• Ideally hospital discharge/ Intermediate care/ discharge to assess experience

• Maintain accurate and timely patient records

 

Skills and Experience Required:

• Experienced working within the NHS (or similar)

• SystmOne experience desirable

• Excellent written and verbal communication skills, including phone and email

 

Essential Requirements:

• Candidates must have NHS experience

• Candidates must hold HCPC registration

• Candidates must hold the right to work in the UK

• Must have car as potential for travelling to other sites and patients’ homes
